Q: I've been losing weight for about a year. I've dropped from a 34-inch to a 30-inch waist size in blue jeans. Unfortunately, I still have a gut that I just can't seem to get rid of. I don't really want six-pack abs, but I do want a flatter stomach. What are some ways to achieve this?

A:  Losing four inches around your middle over a year is solid proof that you're on the right track. But just going by your jeans size alone doesn't give me enough information to assess your weight or body-fat status.

Are you still a little overweight? Or have you reached a normal weight in 30-inch jeans? Or are you underweight with excess ab flab? You don't mention how you lost the weight: Was it through diet alone, or from a combination of diet and exercise? And if you have been working out on a regular basis, have you been doing more cardio or more toning? All these variables can affect your waistline.

Apple shapes—where waist circumference is bigger than hip circumference—carry more health risks than pear shapes, where the hips are bigger. To find out whether you are an apple or a pear, you need to figure out your waist-to-hip ratio: Divide your waist size by your hip width at their widest points. A number higher than 0.80 in women and 0.95 in men suggests that too much fat is carried in the midsection and is linked to having greater health risks.

One thing is clear: You've been very successful at consistently losing weight for a year, something that isn't easy to do. It's natural to reach plateaus where weight loss is stymied while your body adapts to the new, lighter, you. This typically happens after about six months of losing weight, but it may vary. During these periods, it's important to settle into a lifestyle where you focus on maintaining what you've lost, so you don't gain it back.

Whether you can whittle down your waist further is unclear. Even a thin person has more than 30 billion fat cells, and genetics can affect where you store your fat—if you tend to store it in your belly, it may be harder to budge. 

Still, for most people, belly fat is the easiest place to lose it. Research shows that if you lose 5 percent to 10 percent of your overall body weight, you may actually lose up to 30 percent of deep ab fat. That's because ab fat seems to be the most mobile type—there are more fat-burning enzymes and more blood vessels to distribute these enzymes here compared to other areas of the body.

Smart diet and exercise choices may make the difference in your middle:

Food ab-flatteners

Reduce animal fats and trans fats. Those who eat too much saturated and trans fats are more likely to overeat and be insulin resistant, and have higher cholesterol and a larger waist size. So eat more fish than poultry or meats, and choose low-fat dairy. And eat plenty of fruit and vegetables, beans and whole grains.

Avoid alcohol. They don't call it a beer belly for nothing. Alcohol may be more likely to increase ab fat, possibly due to the effects of the alcohol, or possibly because every drink is an infusion of 200 to 400 calories (the equivalent of one to two candy bars), so several nights out a week can really add up.

Eat good carbs. Minimize eating processed, refined carbs like white bread, bagels, muffins, pasta, cookies and cakes; they are linked to larger waistlines. Eat whole grains (that means cooked brown rice, barley, quinoa and the like.) When that's not practical, choose the least-processed versions of foods and try to enhance them. So, choose whole-wheat pasta and sprinkle quinoa in the boiling pot, or choose whole grain (not just "brown") bread and add a layer of uncooked oatmeal to the fillings inside (you won't taste it but it will add fiber, nutrients and more chew to your sandwich or toast).

Fitness ab-flatteners

Do cardio nearly every day. Regular exercise will help you lose more deep abdominal fat, and cardio seems to work best since it burns the most calories per minute.

Work out at higher intensities. Get your heart rate up higher. One study at Duke University in Durham, N.C. found that 175 overweight men and women who walked at least 11 miles per week prevented increases in their ab fat over eight months. (So, about 45 minutes, 4 days per week.) But those who worked harder (walking fast or jogging) for 20 miles a week (about 60 minutes, 5 days a week) actually lost ab fat—and this was all without dieting!

Try for 60 to 90 minutes of cardio a day. Start walking or doing other types of cardio as much as you can. If you're a new exerciser, build up fitness first with 15 minutes of brisk walking or cardio activity every day and add more minutes every week. Work up to doing 60 to 90 minutes on most days of the week.

Don't think skinny people enjoy a cakewalk when it comes to fitness.  Having done both skinny and fat, it's far more motivating to have a lot of weight to lose because the weight shifts off at a faster rate than it does for those of us who are at (or nearly at) a healthy weight but still have unacceptable amounts of flab and jiggly bits you can't see under our compression shorts.  And while it may seem like it takes forever to lose 50 or more pounds, let me tell you it also seems like forever when the dimples and dents I've been battling hang on like a terrified climber on a sheer cliff wall.

 

While you're worried about how to make yourself go into the gym for an hour, I'm worried about Martica's recommendation for me of 90 min of cardo per day!  Ninety?  NINETY????  I'm exhausted just reading that!

 

The bottom line, thick or thin, is that it takes hard work over a long period of time to snap it all into shape.  People with less poundage work just as hard for just as long to firm it all up.  The heavy hitters are already doing the work over time, so when they hit their target weight, they look great!  The lightweights will tell you it isn't about the number anymore, it's about tone, and that takes ages to acquire no matter what your number on the scale.

 

And for those of you larger folk who feel too intimidated to walk into a gym.... I love seeing you there.  I truly do!  You motivate me.  You inspire me.  I'm not looking down on you with scorn from high above on my elliptical trainer as you walk past.  I'm admiring your tenacious nerve and wishing I had it. Open-mouthed 

 

Ninety minutes of cardio.... really?  Ninety?

I know that it is very difficult to lose weight and inches. I used to weight 230 lbs. THrough workouts I was able to lose about 30lbs but I could not loose any more weight. Luckily, I developed a food allergy to corn about a year ago. I was forced to eat very healthy. Now that I eat healthy and exercise on a fairly regular basis, I now weigh 153lbs. I still have another 30lbs I want to loose since I am  only 5' 1" female. I loose about .5-1 inch a week every 2 weeks.

 

My words of advise is: never give up. You will be able to get to your goal weight and size.

 somewhat like MarkMetlerSr says...  lower carbs,, but I suugest more south beach as I think veggies of all make and color are important nutritionally.  The expert here really has it pegged...  Good carbs only and cardio..cardio..cardio..  that doesn't mean running until you drop, just find an hour a day and spend it walking briskly.

 

What have you tried as far as diet an excercise?

Forget about your past dieting/exercise failures for a moment and try to Just focus on one thought Getting BACK UP right now because you CAN DO IT.  Here are few things I suggest ...
The only reason most people fail in their fitness goals is that they have good intentions at first to adopt a new lifestyle, yet after a few weeks or months, they abandon their good intentions and slip right back into their old bad habits that gave them the excess body fat in the first place.

Reducing your body/abdominal fat should be one of your TOP priorities for this year as I am sure you are too Important person in your family and loved ones.

Hi Cobi!  I am sorry to hear of your personal situation right now! I will tell you, when my hubby and I met, I was pushing 170 pounds on a 5 foot 5 inch fame! Within about 6 months, I had lost over 55 pounds! I didn't exercise, I didn't diet or take the diet drugs nor diet pop! What I did was cut out pop all together and started moving around a bit more! My eating habits didn't change...much! I just found that keeping myself busy doing housework and taking care of our kids kept me from sitting around eating on whatever I could get my hands on! If you could bring yourself to eat a bowl of oatmeal for breakfast, that will help you feel full and satisfied for several hours! If you try cutting out sugars by replacing them with sugar substitutes, you're just adding chemicals to your body that your body doesn't know what to do with essentially causing bigger health issues! You actually need the natural sugars to help metabolize your food! Try not buying prepackaged meals, cook at home! We found steaming meats a wonderfully delicious alternative! Keeps the flavor, drains the fat and actually plumps the meat with water!!! The prepackaged foods also contain tons of chemicals your body doesn't want nor need! Make sure you only eat until the hunger is gone.....not until you're full!! If you drink a glass of milk before your meal, you won't eat as much! I also recommend slowing down when you eat! I've been living by "the slower you eat, the less you consume, the less weight you gain"! I know all of this sounds like a major contradiction to what advertising tells us! Keep up the running, or walking, whichever you choose, but give this a try! You will actually lose weight the healthy way with out putting your health at risk by ingesting all the chemicals! I've never before responded to these type things before..  but I feel compelled to share my story.  although I am only 34 I have had similar frustrations about myself at one time...  my wife seemd disinterested but I was 341 lbs at my heaviest.  I have some advice and take or leave it.. it seemd to work for me and I hope it helps you.  I feel that you now have hit rock bottom and are ready like I was to change your life.  I found it easier to keep to my excercise plan by walking instead of running.  Try to walk everyday instead of running three days.  It takes more time but you'll be more apt to continue walking than jogging and it will be less hard on your joints.  I ate less of course but I ate 95 % of my carbs in the morning,  nuts and the like for snacks during the day and salad every lunch followed by a smaller portion of poultry/fish for dinner and more veggies. I know it is easier said than done but I think your on the right track.  Just take your time and realize that it takes time to take it off, but your sex life will improve quicker than you think.   I now weigh 220 lbs and have a very happy misses...  be patient and remember to learn to walk before you run.  Good luck and god bless you
